You Made It To 90. A New Study Asks What Happens To Your Brain Next.

Almost everything we know about dementia risk comes from studying people in their sixties, seventies, and early eighties. Then the research mostly stops — right at the age more and more of us are reaching. A new study followed 805 people who were already 90 or older and asked the question nobody had good numbers for: what happens next?
Why This Study Is Different
Researchers at UC Davis and Kaiser Permanente Northern California built a study called LifeAfter90. Everyone who joined was at least 90 and had no dementia at the time. They were examined every six months, from July 2018 through November 2024.
Two things make this group unusual. The first is age: the middle of the group was 92, and the oldest was 103.
The second is who was in it. Studies of the very old have almost always been made up of white participants. This one was not. Of the 805 people, 26% were Asian, 24% were Black, 20% were Hispanic or Latinx, and 28% were white. Six in ten were women.
What They Found
Dementia does not stop arriving. Over an average of two years of follow-up, 138 people — about 17% — developed dementia. Put another way, the rate worked out to roughly 12 new cases each year for every 100 people. Reaching 90 with a clear mind is a real achievement. It is not a finish line.
Women were at higher risk than men. Women in this group were about 1.9 times as likely as men to develop dementia. That gap looks a lot like the one seen at younger ages.
Differences between groups did not disappear. Black participants were about 1.75 times as likely to develop dementia as Asian participants. The researchers called it striking that the gaps seen in younger adults carry right on into the tenth decade of life. The study measured these gaps. It did not measure why.
The famous risk gene lost some of its punch. APOE-e4 is the gene most often linked to Alzheimer's risk. In this group, people who carried it were not at significantly higher risk than people who did not. The protective version of the same gene, APOE-e2, did still matter — carriers were about 60% less likely to develop dementia.
Years of schooling made no difference. Education is often used as a rough stand-in for a lifetime of mental activity. After 90, it did not separate the people who developed dementia from those who did not.
The Really Good News
Here is the honest part first. This study looked at things you cannot change: your sex, your ancestry, your genes. It did not measure exercise, diet, sleep, or friendship, so it cannot tell you that those things work.
Now look at what it does say. The one inherited factor everybody worries about did not predict who developed dementia after 90. Years of schooling did not either. Two of the easiest reasons to give up — it's in my genes and I never went to college — just got quieter. Something else is doing the separating, and this study did not go looking for it.
Its main recommendation is one you can act on this week. The authors concluded that their findings "reinforce the importance of dementia screening and surveillance, even among people with exceptional longevity." We cannot assume that someone who reaches 90 without dementia is in the clear. So keep the appointments, and say the memory changes out loud.
A Note For BrainSavers Instructors
Two talking points here.
The first is that this study is about the person your participants hope to become. Most brain-health research quietly stops in the early eighties. Someone in your course at 78 has probably never seen a single number that applies to the decade after 90. Now there is one, and it says that decade is still live.
The second is a question you should expect. If someone asks about the differences between groups, keep it simple: the study reported them and did not explain them. Access to care, income, and health history all differ between communities, and none of that was part of this analysis. Say what the study said, and stop there.

If you are in your sixties or seventies, there is real encouragement here. The gene everyone is afraid of did not decide who got dementia in this group. Neither did how far anyone went in school. Those two doors closing quietly is good news for anybody who has ever thought their odds were already fixed.
If you are in your eighties or nineties, the message is shorter and just as good. You are not past the point where this matters. The people in this study were being carefully checked at 92, at 95, at 100 — because at those ages, what happens to your brain is still worth watching.
Three Things You Can Do This Week
Book the check-up you have been putting off. Screening was the authors' own recommendation, and they made it for people at every age above 90.
Say the memory thing out loud. If you or someone close to you has noticed a change, tell a healthcare provider. Do not wait for a birthday to make it official.
Keep showing up. This study did not test daily habits — but nothing in it gives you a reason to stop.
For years, dementia research treated 90 as the edge of the map. Eight hundred and five people just walked past it, and researchers are still asking them, every six months, how they are doing. That is the right question at any age.
